Individual player needs to:
1. Try-on uniforms for sizing (samples provided at Dynamos team meetings).
2. Fill out Soccer 4 All individual selection form.
3. Give completed form to your team manager/uniform coordinator.

Team Mgr./Uniform Coordinator needs to:
1. Collect individual selection forms from players.

2. E-mail dannie@soccer4all.com  OR anita@soccer4all.com to receive team order form.
3. Complete team form and return to above email address. Keep Copies.
4. Arrange pick-up & payment of complete team order by one team rep.

Soccer 4 All

1. Confirms order receipt

2. Completes order - allow minimum 2-3 weeks.

3. Notifies manager when team order is ready for pick-up/payment.

4. Accepts one payment for entire team order in form of credit card or team check.
